What's in the biopsychosocial assessment template

Templates:

• /bps, the full write-up: identifying block with the age computed from the date of birth, presenting problem, biological, psychological and social domains, strengths, formulation, diagnostic impression and recommendations with a computed plan date. Menus for the common findings; choose "described below" to add a description in free form text.
• /social, the social history as one paragraph: living situation, relationship, work, education, supports and stressors from menus that read as full sentences.
• /formulation, a frame for the predisposing, precipitating, perpetuating and protective factors, with the reasoning sentence left to you.
• /strengths, personal, relational and practical strengths as a paragraph, with two optional safety-plan lines.

Lines you often type in assessment, complete and ready to edit:

• /reliable, the sources-and-reliability opener; /attest, the closing attestation; /recs, the recommendations paragraph.
• /denies.sub, /denies.risk, /denies.trauma, /denies.famhx and /denies.legal for the negative findings; /hx.med and /hx.dev for an unremarkable history.

How it works

1. Install the template. Start with /reliable: type it at the top of your next assessment and the opening line appears.
2. Type /bps when the interview is done. Fill the text boxes, pick from the drop-down menus, select the optional sections.
3. Insert. It lands as text you can still edit.

Change the defaults once (name, credentials) and make it your own. Read it as a biopsychosocial assessment example of structure, not clinical instruction: the menus hold the vocabulary and the judgment stays yours.
