Three BIRP note templates for therapists who document in Behavior, Intervention, Response, Plan. Each expands from a shortcut with the four sections laid out and a prompt inside each saying what belongs there, so the progress note comes out specific enough to stand up to a payer review.

What's inside: three BIRP note templates

• birp.note is the quick one. Plain text, client and session details across the top, then the four sections with their prompts.
• birp.form is the billing version. It fills in the place of service and CPT code from the location, session type and duration you picked (90837 from 53 minutes, 90834 from 38, 90832 from 16), and adds ICD-10 diagnoses, treatment goal status and the objectives you worked on.
• birp.guided walks the whole note. Menus for presentation, affect, mood, symptoms and risk, toggles to quantify sleep hours and symptom frequency, an interventions list of nine approaches and fifteen techniques, and a tracking method for the homework you assign.

Written to survive a payer review

Vague notes are what get denied. Every prompt in the quick version carries a BIRP note example of the weak wording, "Client responded well", right where you are about to write the strong version. The guided one asks for a measurable response instead: what the client could do, quantified, and with how much support.

The intervention section is built to carry medical necessity. You name the approach, then name the symptom or treatment goal it targets, so the link a reviewer looks for is on the page.

2. Work down the menus and prompts. You type only what is specific to this session.
3. Your name and credentials carry to the signature line. Check the billing codes against your payer contracts before the claim goes out.
