• dap.note is the quick one. Plain text, a header for client, session date, length, type and location, then Data, Assessment and Plan boxes with the prompts.
• dap.form is the same note as a table, with client details up top and a signature block carrying your credentials.
• The guided version, dap.guided, splits Data into client report, an optional direct quote, observations you pick from menus (appearance, mood, affect, thought process, engagement), session content and homework review.

The placeholders keep Data and Assessment apart

Mixing observation with interpretation is what gets notes sent back. The Data prompt asks for what happened, with none of your read on it. Assessment is where your read goes: working hypothesis, screening results, progress toward goals, and risk, documented even when denied. Plan covers goals addressed, revisions, homework and next steps.

The guided version adds the parts that get missed. Suicidal and homicidal ideation, self-harm, access to lethal means, safety plan status, protective factors and overall risk are menus. There is an ICD-10 field, and a checklist before the signature asking whether the note ties back to the treatment plan and whether someone unfamiliar with the case could follow it.

How it works

1. Add the folder, then type dap.note, dap.form or dap.guided wherever you write notes.
2. Fill the boxes. Menus and toggles cover the repeat wording, so you type only what was specific to this session.
3. Your name carries down to the signature line.
