About this snippets template

OASIS-E cheat sheet and phrase pack for home health

This OASIS-E cheat sheet is a set of 10 snippets for start of care, resumption and recert visits. Type a shortcut wherever you chart and the text appears. You can fill in text boxes and pick from drop-down menus as it inserts, and every word stays yours to edit. A printed PDF still means typing the wording again in the chart.

What's inside

Five full templates

  • OASIS SOC narrative (/socnar) in the order reviewers read it: referral, history, findings, medications, function, homebound status, skilled need and plan
  • Recert narrative (/recert) with progress on each goal and continued need
  • Homebound status justification (/homebound) covering both Medicare criteria, with a text box for patient-specific evidence
  • Face-to-face attestation (/f2f) with encounter date, practitioner and primary diagnosis
  • An OASIS documentation cheat sheet of narrative phrases (/mitem) for ambulation, transfers, bathing, cognition and medications, keyed to item numbers

Five shorter snippets for commonly typed sentences

  • Criterion one and criterion two homebound sentences (/needsaid, /taxing)
  • Skilled nursing need statement (/snneed)
  • Visit frequency order line (/snfreq)
  • Start of care consents and legal paragraph (/soclegal)

Who it's for

RN case managers, PTs and OTs who open cases and recertify in home health. It works in any EHR, because the text goes wherever your cursor is.

Good to know

These OASIS-E assessment phrases are your narrative wording, not the item text. Score items from the current guidance manual, and check homebound and face-to-face wording against your agency's requirements.

Start in two minutes

Use /socnar on your next admission and see how long the narrative takes. Then type /taxing for the taxing-effort sentence, change the wording to match how you write, and save one more sentence you type often as your own snippet. That is where minutes turn into an evening back.
