Veterinary discharge instructions template pack


Three go-home sheets and the paragraphs that fill them, as snippets you can insert where you already type. Type a shortcut, fill in the popup, and the sheet appears with the pet's name, medications and recheck date in place. Set your clinic's instructions and your after-hours number once, and every discharge carries them.

What is inside


Full sheets:

  • /dcgen: the general discharge, with what we did today, medications, food and activity, what to watch for, your clinic and emergency numbers, and a recheck date calculated from the interval
  • /dcspay: the spay and neuter post-op sheet, with a cone or recovery suit menu, the rest period, and a sutures block you switch on when there are skin sutures to remove
  • /dcdental: the after-dental sheet, with an extractions block that adds the soft food period when teeth came out

Shorter snippets for commonly typed paragraphs:

  • /vaxmon the vaccine reaction paragraph, /genmon what to watch at home after a sick visit

How it works


Install the pack, then type a shortcut wherever you write to clients: browser-based practice software, email and text platforms, and installed programs through the Text Blaze desktop app. Fill in the text boxes and pick from the menus as it inserts. Guidance notes show while you fill in the form, never in what the client reads.
