Vet SOAP notes and physical exam templates

What is inside

Templates:

• /vsoap: a full veterinary medical record template with signalment, history, vitals, assessment and plan. Weight converts to kg, the body condition score names its classification, and the recheck interval turns into a date
• /pe: veterinary physical exam template for dogs and for cats, system by system, as plain text you edit down to the abnormals

Shorter snippets for commonly typed sentences:

• /bar the BAR line, /hl heart and lungs, /abdn the abdomen, /eent eyes, ears and mouth with a dental grade menu
• /integ skin and lymph nodes, /neuron the neuro line, /hx the history opener
• /bcoll a blood collection note with a site menu, /ddx the assessment opener, /plan the closing paragraph with a recheck menu

How it works

Install the pack, then type a shortcut wherever you write records. It works in browser-based practice software, in email and client messages, and in installed programs through the Text Blaze desktop app. Fill in the text boxes and pick from the menus as it inserts. Guidance notes show while you fill in the form, never in the record.

The SOAP notes veterinary teams write are never quite the same clinic to clinic, so every line here is plain text you can edit once and keep.
